    The role:

    As a Clinical Nurse Assessor working within a primary health care framework, you will play a key role in evaluating the aged care needs of older people living across Southern Tasmania. This role works collaboratively within a multi-disciplinary team environment to promote wellbeing, and independence.

    The team:

    The Aged Care Assessment Service (ACAS) complete aged care needs assessments under the Aged Care Act 2024.

    You will need

    1. Highly developed knowledge, skills and expertise with advanced competence in evidence-based nursing and a highly developed understanding of the interaction of nursing and other professions in a multidisciplinary setting.
    2. Proven competence in a relevant professional field and a commitment to and understanding of the principles of Primary Health Care, with the ability to incorporate these principles into practice.
    3. Ability to function instructively as a member of an interdisciplinary team with the capacity to act in a leadership role, set realistic goals, evaluate work performance, and provide own self-care.

    Details of appointment

    Permanent, full time, day worker position, working 76 hours per fortnight, commencing as soon as possible.

    *notwithstanding hours to be negotiated with the successful applicant.

    Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

    Salary

    $109,767 to $115,813 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.

    • Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

    In addition, the following allowances calculated on the salaried incremental point may be available:

    • Post Graduate Allowance of up to 10% based on qualification type

    Eligibility

    Successful applicants will be required to meet the essential criteria.

    • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse

    • Current Drivers Licence

    Applicants should note the following criteria are desirable:

    • Postgraduate qualifications, or proven experience, in the assessment and management of clients with aged care issues such as delirium and dementia
